Therapeutic Class: Alpha-adrenergic receptor blocker
Mechanism of Action:
Tamsulosin is a “uroselective” alpha-1 blocker.It targets alpha-1A adrenoreceptors in the prostate and bladder neck, causing the smooth muscles in these areas to relax. This relaxation reduces urinary outflow resistance and improves urine flow, alleviating symptoms of Benign Prostatic Hyperplasia (BPH).
Indications:
- Benign Prostatic Hyperplasia (BPH): Treatment of the signs and symptoms of an enlarged prostate, such as difficulty starting urination, a weak stream, urinary frequency/urgency, and the sensation of incomplete bladder emptying.
- Ureteral Stones: Sometimes used to facilitate the passage of kidney stones through the ureter.
Pack Size: 20 Capsules
Capsules must be swallowed whole; do not crush, chew, or open.
Critical Safety Warnings:
- Intraoperative Floppy Iris Syndrome (IFIS): Patients planning cataract or glaucoma surgery must inform their ophthalmologist if they are taking or have ever taken tamsulosin. The drug affects iris muscles, making surgery more difficult and increasing complication risks. This risk can persist for months or even years after stopping the medication.
- Postural Hypotension & “First-Dose Phenomenon”: Tamsulosin may cause a sudden drop in blood pressure upon standing (orthostatic hypotension), leading to dizziness or fainting (syncope). This is most common after the first dose or when restarting treatment.
- Priapism: Rarely, tamsulosin is associated with priapism—a painful, persistent erection unrelated to sexual activity. This is a medical emergency that can lead to permanent impotence if not treated immediately.
- Prostate Cancer Screening: Symptoms of BPH and prostate cancer can be identical. Patients should be screened to rule out prostate cancer before starting and periodically during treatment.
- Sulfa Allergy: Use with caution in patients with a history of sulfa allergy, as rare cross-allergic reactions have been reported.
- Severe Skin Reactions: Though rare, serious skin conditions such as Stevens-Johnson Syndrome have been reported. Discontinue use immediately if a rash or swelling occurs.
