Description

Therapeutic Class: Immunosuppressant (Calcineurin Inhibitor).
Mechanism of Action:
Inhibits T-lymphocyte activation by binding to FKBP-12 and inhibiting calcineurin. This blocks the production of interleukin-2 (IL-2), preventing the immune system from identifying and attacking foreign or “self” tissues.
Indications:
Tacrolimus, available as Taclekin, is primarily employed post-organ transplant to prevent rejection. It achieves this by inhibiting the immune system’s response to the transplanted organ, thus ensuring its acceptance by the body. Commonly used in kidney, liver, heart, and other organ transplants, Tacrolimus is integral to long-term therapy, maintaining the organ’s viability.While occasionally prescribed for certain autoimmune disorders like lupus nephritis, its usage demands careful monitoring due to potential side effects and the need to maintain therapeutic blood levels.

Critical Safety Warnings:

  • Grapefruit Warning: Patients must strictly avoid grapefruit and grapefruit juice, which can dangerously increase tacrolimus levels to toxic amounts.
  • Nephrotoxicity (Kidney Damage): Tacrolimus can cause acute or chronic kidney impairment. This risk is increased when taken with other nephrotoxic drugs like ibuprofen or certain antibiotics.
  • Neurotoxicity: Patients may experience severe neurotoxicities, including seizures, tremors, delirium, and a rare brain condition called PRES (Posterior Reversible Encephalopathy Syndrome).
  • Cardiac Risks: Myocardial hypertrophy (enlargement of the heart) and QT prolongation (heart rhythm changes) have been reported, especially with high blood concentrations.
  • Interactions: Tacrolimus and cyclosporine must not be used at the same time; one must be discontinued at least 24 hours before starting the other
