Mechanism of Action:
Sevelamer contains multiple amines that become partially protonated in the intestine and interact with phosphate molecules from food through ionic and hydrogen bonding.This interaction results in the binding of dietary phosphate in the gut, forming an insoluble complex that is then excreted in the feces.Because it is not systemically absorbed, its effect is localized to the gastrointestinal tract, lowering serum phosphate concentrations without altering serum calcium levels.
Indications:
- Hyperphosphatemia Management: Control of serum phosphorus in patients with chronic kidney disease (CKD) who are undergoing dialysis (either hemodialysis or peritoneal dialysis).
- Its use helps prevent the complications associated with high phosphate levels, such as bone disease (renal osteodystrophy) and ectopic calcification.
Critical Safety Information:
- Administration: Must be taken with meals to bind dietary phosphate effectively.Tablets must be swallowed whole.
- Contraindications: Not for use in patients with bowel obstruction or hypophosphatemia (low phosphate levels).
- Drug Interactions: Sevelamer can bind to other oral medications (e.g., ciprofloxacin, mycophenolate mofetil, levothyroxine, tacrolimus) and reduce their absorption. These medications should be taken at least two hours before or six hours after sevelamer.
