Mechanism Of Action:
Mycophenolate Mofetil functions as a potent, selective, and reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH). This enzyme is essential for the de novo synthesis of guanosine nucleotides. Because T and B lymphocytes rely almost exclusively on this specific pathway for DNA replication , unlike other cell types that can use “salvage” pathways , this action selectively suppresses lymphocyte proliferation and prevents the production of antibodies, effectively dampening the immune response.
Indications:
Mycophenol is typically used as part of a multi-drug regimen (often with cyclosporine and corticosteroids) to prevent rejection in kidney, heart and liver transplant patients.It is also used in autoimmune conditions such as lupus nephritis, rheumatoid arthritis, connective tissue disorders and other dermatological conditions.
- Handling: Tablets must be swallowed whole; do not crush or chew. The active powder is hazardous if inhaled or if it comes into contact with the skin or mucous membranes.
- Embryo-Fetal Toxicity: Use during pregnancy is associated with an increased risk of first-trimester pregnancy loss and severe congenital malformations (birth defects). Women of childbearing potential must use effective contraception and have a negative pregnancy test before starting therapy.
- Serious Infections: Patients are at increased risk of developing opportunistic infections, including fatal bacterial, fungal, or viral infections.
- Malignancies: Long-term immunosuppression increases the risk of developing lymphomas and skin cancers. Patients should limit sunlight and UV exposure.
- Gastrointestinal Risks: Use with caution in patients with active digestive system disease, as it may cause gastrointestinal ulceration, hemorrhage, or perforation.
- Drug Interactions: Absorption is significantly reduced if taken with antacids (magnesium/aluminum-based) or phosphate binders (e.g., Sevelamer). These should be taken at least 2 hours apart.
