Mechanism of Action: Fosfomycin works by inhibiting a specific early stage of bacterial cell wall synthesis. It inactivates the enzyme enolpyruvyl transferase, which is critical for bacterial survival and replication. This unique mechanism generally means there is no cross-resistance with other classes of antibiotics.
Indications:
Fosfomycin is primarily indicated for single-dose therapy:
Uncomplicated Urinary Tract Infections (UTIs): Treatment of Lower urinary tract infections (cystitis) in women (18 years of age and older).
Susceptible Bacteria: Effective against common susceptible urinary pathogens, including Escherichia coli and Enterococcus faecalis.
Critical Safety Information:
Administration: The entire contents of one 3g sachet should be dissolved in about 4 ounces (120 mL) of cool water and taken immediately.
Single-Dose Treatment: It is highly effective as a single dose for uncomplicated UTIs.
Limitations: This medication is not indicated for treating pyelonephritis (kidney infection), complicated UTIs, or infections in men, where a multi-day course of antibiotics is typically required.
Common Side Effects: Common side effects are generally mild and may include diarrhea, headache, nausea, and vaginitis.
Contraindications: Not for use in patients with known hypersensitivity to fosfomycin. The product contains sucrose and should be used with caution in diabetic patients.
